If you need to upload your child's immunization records, please follow these steps.
- After you log in click enrollments at top right side of the page
- Click order history
- Find the most recent order at the top of the list and click the down arrow to the left of it
- Click the 3 dots on the right side under the “actions” column
- Click registration form
- Click edit
